Wholesale Registration Information
To register for a wholesale account with Abundant Health, please read the Wholesale User Agreement, and then fill out the Wholesale Account form below and fax the following to Abundant Health:
- A copy of your current business licence
- A completed Utah State Tax Exemption Certificate (Download).
Once we receive this information, we will review your request, and if approved, we will activate your account and email you your wholesale account information within 1 business day. You may then use your wholesale log-in information to access Abundant Health's wholesale section. If you have any further questions, please don't hesitate to call us at 1-888-718-3068.

By filling out the form below or faxing your information to Abundant Health to complete the registration process, you agree to the following terms of use:
- Abundant Health reserves the right to deny any request for access to Abundant Health's wholesale section at its sole discretion.
- If approved, you will be issued a user-name and password that will allow you to access the Abundant Health wholesale section. You may not distribute this information to others, or allow others to use this information to access the wholesale section of our web site.
- Failure to abide by these terms will result in the termination of your right to access the Abundant Health wholesale web page and to receive wholesale pricing.
